Ride/Park Accidents

Somewhere in international waters, two suffer injuries when the bottom of a drop slide splits open. Some miracle noone fell from height.

looks not too serious (interms of injury) but a pretty big screw up:

looking at it, there are a few options that could have happened, however I can't imagine it not being one of these issues:
track out of guage, allowing up stop wheels through and above the track
up stop wheel mounts bent (happened on a similar type of ride in florida if I recall correctly)
up stop wheels / assembly detached / not present

RtRM's take on things.
He seems to think the trim brakes on the drop must have been ripped up by the train's fin jamming in it somehow. Certainly seems plausible to me.

Yeah just saw that, I didn't see the brake earlier, I would certainly have to agree as it seems like the most likely scenario.

even if it was something else (such as being derailed before hitting the brakes (see disney BTM derailment)) it rases questions about the maitanence of the ride
